Phyllis Formby

Phyllis Formby, 94, passed away on May 2, 2020. A Celebration of her Life will be held 10:00 a.m. Thursday June 25, 2020 at Stinnett Cemetery She was buried by her husband Buck in a private ceremony.

Phyllis was born February 18, 1926 in Mounds, Oklahoma to Kit McKinley Baker and Versa Esther Thorn Baker. She has lived in this area since 1940 graduating from Phillips High School in 1943 and had a degree in Zoology from Texas Tech University. She was a Licensed Vocational Nurse working at several nursing homes. Phyllis was a dedicated and loving person. She was active in helping those that were unable to help themselves.

She was preceded in death by her parents, her husband Buck and her brother James McKinley Baker.

She is survived by her son Bill Formby of Amarillo and her daughter Toni Formby of Dallas.
